Instructions for Completion of Forms
Personnel Action Notice
- Affix date of completion to the form.
- Complete ALL 4 blanks at the top of the form.
- Please select ACTION (i.e. New Hire, Transfer, Returning Adjunct, etc.)
- Please complete the "Current Data" and "New Data" tables in their entirety (if possible) if the person is a new faculty or staff member.
- Obtain approvals of Manager and Dept. Head/Dean then forward to the Office of Academic Affairs. This office will forward the form to the Vice President for Business & Finance.
- If the PAN is missing the Effective Date, BCAT Code/Title, Position #, Replacing (if applicable), Salary, Account #, EFT, Budget EFT, and/or Budget Salary, the PAN will be returned to the Administrative Dept. Head or the Academic Dean.
Records Destruction Form
- Using the University System of Georgia Records Series Index (http://www.usg.edu/usgweb/busserv/series/) determine which records should be destroyed.
- Complete the NGCSU Certificate of Records Destruction (http://www.ngcsu.edu/Resource/Forms/certrecdestruc.pdf).
- Send the completed NGCSU Certificate of Records Destruction to Laura Mullins for approval to proceed with the records destruction.
- Academic Affairs will returned the approved form to you.
- Destroy the records, sign the NGCSU Certificate of Records Destruction that the records have been destroyed and return the form to Laura Mullins in Academic Affairs.
- Failure to follow this procedure could result in significant legal problems for the university. Please be sure you keep your records destruction process up to date.
