How to Apply as a Degree Seeking Student
1. Complete the online application for graduate
admission. Students who choose to apply using the
paper application should submit the completed
application form and a check or money order made payable to
NGCSU in the amount of $40. The application fee is
one-time, non-refundable.
International students (requiring a F1 visa) must
complete the application for international
students.
2. Schedule a graduate
entrance exam required for your program. It is
recommended that the exam be taken 6 weeks prior to the
application deadline to insure adequate time for processing.
The scores should be reported directly to the Office of
Graduate Admissions by selecting North Georgia College &
State University, institutional code #5497. Photocopy of
your personal test report may be submitted to verify that
the exam has been taken; however, official test results must
be received from the Educational Testing Service before your
application is considered complete, and test scores can be
no more than six years old. If your program will
accept and you choose to take the Miller Analogy Test (MAT)
through the NGCSU testing office, you will receive an
unofficial score report day of test which we can use to process
your application if you hand-carry the report to the
Graduate Admission Office, Room 102, Price Memorial.
It does require three weeks for the official
score report to be mailed to the Office of Graduate
Admissions.
3. Request
official transcripts from each institution attended be
sent to you (do not open) for inclusion in your admission
packet or request the transcripts be sent directly to the
Office of Graduate Admissions at NGCSU. If you graduated
from NGCSU, you do not have to request official transcripts
(DPT candidates will need to submit official NGCSU final
transcript if accepted in program).
4. Download the
recommendation form and have three recommendation forms completed and returned directly to the Office
of Graduate Admissions.
5. Download the NGCSU
certificate of
immunization and have completed by health care provider.
Note: (DPT applicants should complete the Medical
College of Georgia immunization certificate.)
As a prerequisite to registration, the University System of
Georgia (Board of Regents' Policy 407.05) requires that all
students provide
proof of immunization to
measles, mumps, and rubella, varicella (chicken pox - not
required if born in the US before 1980) are also required to provide proof of immunization
to varicella, tetanus, diphtheria, and if 18
years of age or less at the time of classes, hepatitis B. Former
NGCSU students are required to meet new immunization
requirements.
If your records are not available, a titer test (blood
test) available through your physician or local health
department may be used to confirm the required immunities to
measles, mumps, rubella, and chicken pox. There are
exemptions for religious or medical grounds and for totally
distance learning programs or courses.
6. Check the program web site for any
additional forms and requirements.

All documents received are kept in the Office of Graduate
Admissions for a maximum of one year for incomplete files
and for a maximum of three years for those accepted but do
not matriculate.
