ImmunizationAs a prerequisite to registration, the University System of Georgia [Board of Regents Policy 408.02] requires that all students provide proof of immunity to measles, mumps and rubella. Effective January 2005, new students are also required to provide proof of immunization to varicella (chicken pox), tetanus, diphtheria and, if 18 years of age or less at the time of classes, hepatitis B. Please click here to view and print the North Georgia Certificate of Immunization form*. Although admission decisions will be made without the North Georgia Certificate of Immunization form, no student will be allowed to register for classes until the Office of Undergraduate Admissions or Cadet Recruitment Center has received this form. Because of the possibility that you may be required to receive a series of vaccinations, we strongly encourage you to begin making arrangements to have the North Georgia Certificate of Immunization form completed by a health official a few months prior to the beginning of classes. Medical records cannot replace the form. Additionally, cadets participating in the ROTC program must have the North Georgia College & State University Medical Report* before enrollment. Information provided on the form is especially important in those cases where the student has some condition requiring special medical assistance. * Note: These forms are in Adobe Acrobat format (.pdf).
