• candidates should apply separately to each school of interest in the DPT Consortium
   
• applicants who choose the online application service through CollegeNet should not select the CollegeNet online recommendation service
   
• applicants must submit the common recommendation form for the DPT consortium which is paper form only
   
• candidates are required to submit a Medical College of Georgia Certification of Immunization Form upon acceptance in the program
   
• applicants should also submit
    official transcripts from each institution attended (including NGCSU)
    official GRE scores
