Currently the IIT Data Management Group does not process any requests submitted by phone or email. The Data Management Group will process only requests submitted through the online Data Management Request form. The IIT Data Management Request form can be accessed from Self-Service Banner. When you log into Self-Service Banner go to Reporting Services and find the form under the Data Management Menu.
FORM PURPOSE: This form was created to enable users campus wide to submit their requests to the Data Management group online.
FORM DESCRIPTION: IIT Data Management request submission.
Once a user submits a request:
1. When the request is submitted successfully a final confirmation page will be displayed with a request id and date of submission. The request id is a unique identifier for the request to be used for tracking the request status.
2. The user will receive a confirmation email containing the request id.
3. IIT will contact the requestor in a timely manner.
