To nominate a candidate for the Excellence in Teaching Awards:

1. Obtain the candidate's permission.

2. Complete a nomination form along with a 500-word or less explanation of why you are nominating the candidate.
   *NOTE: Each candidate must be nominated by two individuals, one of whom must be a student.
